Nursing Grand Rounds - Heart Failure Patient Education: The Integral Role of the Nurse

Heart Failure is a growing endemic with numbers expected to rise to 8 million by the year 2030. It is a multifarious diagnosis often accompanied by a number of comorbidities. Patients are often high acuity and medically complex, requiring multi-disciplinary collaboration and management. Nurses are integral members of these patient’s care teams and their only continuity. Given the complexity and severity of their illness, Heart Failure patients are at greater risk for gaps in care including, frequent re-admissions. This session will provide a brief overview of HF management as well as shine light on the crucial role of nurses in the care of patients with HF. 

Learning Outcome(s)

At the conclusion of this learning activity, (at least 75% of) participants, using AAHFN guidelines, will be able to apply lessons learned regarding care of the patient with heart failure, in order to provide effective patient and family education that will ultimately enhance patient outcomes.
