Submitting or Updating Your Financial Disclosure Form

1. Login to access your Tasks



If you are unable to sign in, please try resetting your password by clicking the Forgot Your Password button. If you are unable to reset your password, email [email protected].

2. Go to My Tasks

3. The Global Tasks area displays assigned system tasks, such as submitting a financial disclosure.



To start a Financial Disclosure Form, select the Begin button.

4. Read the introductory text, answer the questions, type in your name and Submit the form.
