Overview
This session will highlight key components of the Telephone Triage Competency Assessment program, focusing on strengthening clinical practice through structured evaluation and meaningful feedback. Participants will review critical elements of the competency designed to identify practice gaps, reinforce best practices, and recognize strengths in ambulatory nursing care. The session will introduce the prototype feedback and audit index used for documentation review, offering practical guidance on consistent, high-quality competency assessment. In addition, early exploration of compassionate presence in telephone triage will be discussed, emphasizing the role of communication, empathy, and patient-centered connection in brief, remote encounters. This session is intended for nurse leaders, educators, and clinicians involved in competency development, quality improvement, and telephone triage practice.

Learning Objectives
At the conclusion of this learning activity, participants will be able to:

  1. Identify key criteria for a high-quality telephone triage assessment, including comprehensive symptom evaluation, recognition of urgent and emergent indicators, and appropriate documentation standards.
  2. Describe strategies for using plain language and teach-back during telephone encounters to improve patient understanding, adherence to the plan of care, and recognition of worsening symptoms.
