Overview

Palliative care clinicians manage complex symptoms, work with patients and families to establish goal aligned care and strive to improve the quality of life for those with serious illness. Most clinicians can provide primary palliative care for those with serious illness and at the end of life. In Year 4 of the Interprofessional Palliative ECHO® program, we will expand on pertinent topics to look at the palliative care in situations and settings through didactic presentations. Clinicians will acquire skills and establish professional connections to improve the care of patients with serious illness and at the end-of-life. The 9 session Palliative Care ECHO 4.0 contains didactics on a variety of Palliative Care Topics. Each didactic is 15 minutes long and will count toward 0.25 CE credits.

Sarah Guarda, LICSW - Dartmouth Hitchcock Medical Center – Trauma Informed Care

Sarah Guarda is a Social Worker on the Inpatient Palliative Care team at Dartmouth Hitchcock Medical Center. She received her Master of Science in Social Work at Columbia University in New York, specializing in both Clinical Practice and Non-Profit Administration. Sarah joined the DHMC team in January 2024, where she utilizes a trauma informed lens to supporting seriously ill patients and their care partners on our service.

Sarah received a Bachelor’s in Psychology from Rutgers University in New Brunswick, NJ. She has over a decade of experience in various areas of social work practice- including palliative medicine and hospice, mental health and wellness, and policy geared towards creating safe and affordable supportive housing. She is passionate about the intersection of advocacy and social justice in health care, and translating this approach into practice. Sarah is a Licensed Independent Clinical Social Worker in both Vermont and New Hampshire.

Phil Lawson, MD - Palliative Medicine Consultant and Emergency Department Physician at Littleton Regional Healthcare - Palliative Care in the Emergency Department

Phil Lawson, MD is a practicing Palliative Medicine Consultant and  Emergency Department Physician at Littleton Regional Healthcare. He was the lead clinician representing the New Hampshire Health Care Decisions Coalition that amended RSA 137:J in 2021, the NH law regarding Advance Directives, POLST, DNR and surrogate decision making; and he, among other things, continues to teach on a state wide basis regarding Advance Care Planning and POLST. Consistent with his 96 yo teacher/father, he likes to be a little controversial as a strategy to stimulate thinking.

Bradley Eckert, MD - Dartmouth Health – LGBTQIA+ Hospice and Palliative Care

Bradley Eckert is a physician in the Section of Palliative Medicine at Dartmouth Health, where he works in the outpatient clinic and at the Jack Byrne Center for Palliative and Hospice Care. He most recently completed his hospice and palliative medicine fellowship at DHMC. Before that, he trained in Ohio in Family Medicine. He also holds a graduate degree in health policy from The George Washington University in Washington, DC and continues to be engaged with both state and federal policy initiatives, including serving on the Public Policy Committee for the American Academy of Hospice and Palliative Medicine and the Legislative Committee for the New Hampshire Medical Society. Outside of medicine, he enjoys spending time with his husband, two very crazy Labrador retrievers, and exploring new hikes in Northern New England.

Diana Barnard, MD - Associate Professor of Family Medicine at the University of Vermont Medical Center- Medical Aid in Dying

Dr. Diana Barnard is an Associate Professor of Family Medicine at the University of Vermont Medical Center. She is also board certified in Hospice and Palliative Medicine. Dr. Barnard is a founding board member of Patient Choices Vermont which led the grassroots effort to pass Vermont’s Medical Aid in Dying Law. Since passage of the law in 2013, Dr. Barnard has worked across the country on both educational and legislative efforts related to Medical Aid in Dying. She believes that everyone deserves the best possible genuinely patient centered end of life care.

H. Samuel Landsman, MD - Director of the Behavioral Intervention Team (BIT), the Inpatient Proactive Psychiatry Consultation Service at Dartmouth Health - Severe Mental Illness and Palliative Care

Dr. Landsman attended Tulane University School of Medicine in New Orleans, LA and completed residency training here at Dartmouth-Hitchcock Medical Center in general psychiatry, and fellowship training in Consultation-Liaison Psychiatry at Brown/Butler/Rhode Island Hospital in Providence, RI. Sam is now working as the director of the behavioral intervention team (BIT), the inpatient proactive psychiatry consultation service at Dartmouth-Health in Lebanon, NH. He is an assistant professor of psychiatry at Geisel School of Medicine at Dartmouth.  Sam is passionate about his work in the general medical hospital with people who struggle with mental health issues.

Amelia Cullinan, MD - Interim Section Chief Section of Palliative Medicine, Dartmouth Hitchcock Medical Center - Serious Illness Conversation Guidance in Dementia

Dr. Amelia Cullinan is the Interim Section Chief of Palliative Medicine at Dartmouth Hitchcock Medical Center, and an Associate Professor at the Geisel School of Medicine. She’s been at DHMC since 2012. She received her medical degree from Jefferson Medical College in Philadelphia, and did an Internal Medicine-Primary Care residency at St. Vincent’s Hospital in New York City, where she also completed a year as Chief Resident. She completed her training in Hospice and Palliative Medicine at the Harvard Palliative Care fellowship program in 2008. Dr. Cullinan’s non-clinical passions are education and quality improvement work. She is one of the core faculty for the Internal Medicine Residency, leading the 2nd year FIRE (Formal Instruction of Residents as Educators) rotation. She also co-directs the Serious Illness Care Program at Dartmouth-Hitchcock, which has educated over 500 interdisciplinary learners in the Serious Illness Conversation (SIC) Guide, and she was responsible for embedding the Guide into D-H’s electronic medical record.

Connolly Huddleston – Board Certified Chaplain, University of Vermont Medical Center-Spirituality and Cultural Sensitivity in Serious illness

Connolly Huddleston is a board-certified chaplain who currently serves as a Palliative Care Interfaith Chaplain at the University of Vermont Medical Center. She received her master’s in Pastoral Studies from Seattle University in Seattle, WA and completed her Clinical Pastoral Education residency at Legacy Emanuel Medical Center and Randall Children’s Hospital in Portland, OR in 2018. Connolly enjoys being in the great outdoors, hiking and traveling.

Dax Volle, MD - Board-Certified Geriatric Psychiatrist at Dartmouth Hitchcock Medical Center and Assistant Professor of Psychiatry and Medical Education at the Geisel School of Medicine at Dartmouth

Dax is a board-certified geriatric psychiatrist at Dartmouth Hitchcock Medical Center and an assistant professor of psychiatry and medical education at the Geisel School of Medicine at Dartmouth. At Dartmouth, he maintains an active clinical practice, supervises residents and fellows, serves as the section chief of geriatric psychiatry, as the program director for the geriatric psychiatry fellowship program, and as the director of the second-year course in psychiatry at the Geisel School of Medicine with an overall goal of coordinating cognitive and behavioral care services for older adults across multiple specialties and clinical sites. His main clinical and academic interests are cognitive health in aging, dementia care, movement disorders, late life depression, palliative care, medical education, neuromodulation, and philosophy of psychiatry.

Hannah Reuman, MD - Adult and Pediatric Palliative Care Provider, Dartmouth Hitchcock Medical Center – Introduction to Pediatric Palliative Care

Hannah Reuman is an adult and pediatric palliative care provider at Dartmouth Hitchcock Medical Center. She completed her pediatrics residency and hospice and palliative medicine fellowship at the University of Pittsburgh Medical Center. She then completed a master’s in medical education before moving to New Hampshire with the hope of creating a pediatric palliative care program at Dartmouth Hitchcock. Currently, there are no formalized pediatric palliative care programs in the state of New Hampshire. She is grateful to champions within the section of palliative medicine and within Dartmouth Medical Center for their support in making this vision become a reality!

Jennifer Ebbitt, RN - Nurse Navigator Jack Byrne Center Dartmouth Hitchcock – Introduction to Pediatric Palliative Care

Jennifer Ebbitt is a certified hospice and palliative care RN in both adult and pediatrics. After more than ten years working for a non-profit hospice organization in Rhode Island, she joined the palliative care team at Dartmouth Health as the Nurse Navigator for their Jack Byrne Center. Jennifer has experience working collaboratively to develop pediatric hospice programs, grow and support a hospital-based inpatient Hospice program, and also worked as a direct caregiver for patients receiving hospice care. She is passionate about contributing her expertise to education and advocacy for interprofessional colleagues, aiming to aid them in providing high-quality, meaningful care for patients of all ages facing serious illnesses and end-of-life care.

At the conclusion of this learning activity, participants will be able to:

  1. Discuss at least two ways to enhance care of patients with serious illness including improved management of symptoms and ability to better address goals of care with patients.
