Overview

This engaging session explores case-based teaching in clinical education, emphasizing authentic patient perspectives, empathy, and inclusivity. Participants reflect on the benefits of real clinical cases for fostering critical thinking, engagement, and active learning. The discussion highlights balancing structure with flexibility, using learning objectives to guide effective case presentations, and incorporating clinical images for clarity and memorability. Educators learn strategies to encourage learner ownership, navigate the complexity of clinical reasoning, and normalize its challenges. Through guided discussions and supportive environments, case-based teaching becomes a powerful tool for deepening understanding, enhancing clinical reasoning skills, and building compassionate, patient-centered healthcare professionals.

Kelly Kieffer, MD, MS

Kelly Kieffer has been a member of the Geisel School of Medicine faculty in the section of General Internal Medicine for over 25 years, and currently serves as Vice Chair for Education in the Department of Medicine. She developed an affinity for case-based teaching and learning as a student in the Joint Medical Program of the University of California, Berkeley and UC San Francisco, where she earned her medical degree. She completed her residency and chief residency at DHMC and earned a Master of Science from The Dartmouth Institute for Health Policy and Clinical Practice.

In her prior roles as Associate Program Director in the internal medicine residency and Program Director of the primary care track, Dr. Kieffer developed multiple enduring and impactful curricular innovations related to ambulatory practice and medical education. She had the privilege of working in Kigali, Rwanda, for a year through the Human Resources for Health program, building and implementing new clinical teaching programs, including morning report, morbidity and mortality conference, team-based learning, and clinical skills conferences as part of an effort to re-establish the University of Rwanda’s internal medicine training program. As the Director of DHMC’s Department of Medicine Morbidity, Mortality and Improvement conference for the past 10 years Dr. Kieffer has supported residents in developing hundreds of case presentations and facilitated rich and robust discussions by members of our community. She is continually humbled by how much we have to learn from our patients’ stories and from each other.

Learning Objectives
At the conclusion of this learning activity, participants will be able to:

  1. Identify features of clinical cases that offer valuable opportunities for learning and reflective practice.
  2. Develop and deliver case presentations that engage learners using strategies aligned with learning objectives and learner level.
  3. Apply clinical reasoning skills to analyze, synthesize, and solve clinical cases through structured case discussions.
