The role of Case Management is often not well understood. Complexities and acuities of our patients are not easily captured in quantitative ways. This session will focus on the crucial role of our Case Managers.

1. Discuss the role and value of the RN Case Manager.
2. Identify system supports across the continuum of care.
3. Explain the tenets of the Donabedian model, including how use of this model provides RNs the opportunity to define and measure their practice as it relates to patient quality outcomes.
