LATE Goals of Care Training: Advanced Skills using the REMAP Framework

AMA PRA Category 1 Credits™ (2.75 hours), ANCC Nursing Contact Hours (2.75 hours), CME-Designated Participation Credit (2.75 hours)

Join us for an interactive, skills-based training in VitalTalk’s foundational REMAP course.

We welcome participants of any discipline: nurses, social workers, physicians, associate providers, and others. We encourage clinical teams to train together.

This conference will be offered virtually using the Zoom platform.

To take part in this learning, participants must have:

• A microphone and a camera
• Zoom downloaded onto a desktop or laptop (not handheld devices)
• High speed internet (>10 MPS)
• A place relatively free from interruptions and loud noise

1. Describe the steps in the REMAP mnemonic for late goals of care conversations.
2. Use the REMAP mnemonic with a patient or surrogate.
3. Create an individualized action plan to incorporate REMAP into their practice.

The registration fees are as follows:

• Physicians and Advanced Practice Providers: $100.00
• All other healthcare professionals, including medical Students: $25.00

The virtual conference meeting access link will be emailed to registered participants 24 – 48 hours prior to the start of the activity.

In support of improving patient care, Dartmouth Health is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Dartmouth Health designates this Live Activity for a maximum of 2.75 AMA PRA Category 1 Credits^TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

American Nurses Credentialing Center (ANCC)
Dartmouth Health designates this Live Activity for a maximum of 2.75 ANCC contact hours. 

All other learners may claim CME-designated participation credit. Consult your professional licensing board regarding the applicability and acceptance of CME-designated participation credit for programs certified for credit by organizations accredited by Joint Accreditation for Interprofessional Education.
