EARLY Goals of Care Training for Cardiovascular Medicine: Foundational Skills using the Serious Illness Conversation Guide

AMA PRA Category 1 Credits™ (2.75 hours), ANCC Nursing Contact Hours (2.75 hours), CME-Designated Participation Credit (2.75 hours)

This three-hour interactive, skills-based session gives healthcare professionals of any discipline a talking map for effective, efficient goals of care conversations. Led by Dartmouth Health Serious Illness Care Program faculty, the session features active learning and reflection, including progressive skills practice. Participants will leave the session with the skills and tools to begin having these conversations with their patients. To maximize time for skills practice, participants will be expected to watch a short video (8 minutes) before the session.

Target Audience
Any healthcare professional who Dartmouth Health Cardiovascular Medicine employs. Also, Dartmouth Health Hospitalists are invited to join.

1. Describe the rationale for the structure of the Serious Illness Conversation Guide including its benefits.
2. Apply recommendations suggested in the Serious Illness Conversation Guide in their patient interactions.

The registration fee is waived. This event is limited to Dartmouth Health Cardiovascular Medicine employees and Dartmouth Health Hospitalists.

Advance registration is required for planning purposes. Please make every effort to register seven (7) business days in advance of the learning activity. We will not accept walk-in or late registrants on the day of the learning activity.
