Overview

This session will offer two examples of creating a community-wide approach to perinatal mental health. Our AIM implementation team and the NHPQC will briefly share administrative updates, details from the Perinatal Mental Health Conditions (PMHC) Patient Safety Bundle, and relevant data. We will then hear from NH Dept of Health and Human Service's Home Visiting Program Manager Paula Gyurcsan, M.Ed and Mid-State Health Family Practice Provider Cecilia Disney, MD who will share learnings from their perinatal home visiting work. The speakers will walk through the design and implementation of their community initiatives before opening up a Q&A session with attendees.

Learning Objectives
At the conclusion of this learning activity, participants will be able to:

  1. Describe key reasons for incorporating home visiting work into postpartum care.
  2. Identify one new approach to offering mental health support in the postpartum period that you can implement in your own professional practice.
