Neuroscience Education Series - Case Management in the Neuro Population

This education series is intended to enhance interdisciplinary communication while addressing the knowledge gaps surrounding Case Managements role in the care of neuro patients.

1. Describe Care Management’s role in coordinating care and resources for patients with neurological conditions.
2. Identify strategies for effective collaboration between nursing and Care Management to support coordinated, patient-centered care.
